Mental Health Crisis Support

All NDIS Participants can access this service.

At Two Hearts QLD, we provide structured NDIS mental health crisis intervention for participants experiencing acute emotional, behavioural, or situational distress.

 

Crises can arise suddenly and significantly impact safety, stability, relationships, and daily functioning. Our role is to respond promptly, assess risk, stabilise the situation, and implement practical supports that reduce escalation and restore safety.

 

A core focus of our service is hospital diversion — supporting NDIS participants to remain safely in the community wherever clinically appropriate.

Same-Day Crisis Response (7:00am – 9:00pm)

We offer same-day NDIS crisis support between 7:00am – 9:00pm (AEST).

Structured Risk Assessment & Triage

All crisis referrals begin with a structured mental health risk assessment conducted via phone.

 

Our triage process determines:

  • Immediate risk level

  • Suicide or self-harm risk

  • Behavioural escalation

  • Mental health presentation

  • Environmental safety factors

  • Appropriate mode of intervention

 

Following assessment, crisis intervention is delivered via:

 

  • Telehealth (available Australia-wide)

  • In-person NDIS crisis support where clinically appropriate

 

This ensures safe, responsive, and clinically appropriate mental health crisis care.

In-Person Crisis Support Locations

In-person NDIS mental health crisis intervention is available in:

  • Brisbane (North and South)

  • Gold Coast

  • Scenic Rim

 

Outside these areas, participants can access telehealth crisis support anywhere in Australia.

What Crisis Intervention Includes

Our NDIS crisis intervention service may include:

 

  • Immediate risk and safety assessment

  • Suicide response planning (non-emergency)

  • Behaviour crisis stabilisation

  • Safety planning for participants and families

  • Strategies to prevent placement or family breakdown

  • Short-term intensive mental health support

  • Coordination with Support Coordinators and allied health teams

  • Advocacy within the NDIS during urgent situations

 

This is short-term, targeted intervention focused on stabilisation, safety, and restoring functional capacity.

Hospital Diversion & Community Stabilisation

One of our primary goals is to reduce unnecessary hospital admissions for mental health crises.

 

We work to:

  • Stabilise participants within their home or community environment

  • Reduce repeat emergency department presentations

  • Strengthen support systems

  • Improve stakeholder communication

  • Develop structured crisis response plans

 

Our team has extensive experience supporting NDIS participants with complex mental health presentations and behaviours of concern.

When Emergency Services Are Required

While our aim is to provide structured community-based NDIS crisis support, there are situations where emergency services must be contacted.


We will direct you to call 000 if:

  • The person is under the influence of alcohol or other drugs (AOD)

  • The person has already acted on suicidal thoughts or plans (e.g., overdose)

  • The person is actively harming themselves or others

  • The person is experiencing acute psychosis (e.g., hearing voices, not oriented to reality)

  • There is immediate danger to life

 

These situations require emergency medical or psychiatric intervention.

Referrals

Referrals are accepted from:

  • Support Coordinators

  • Plan Managers

  • Allied Health Professionals

  • Participants and families
