NDIS Mental Health Support Coordination

Support Coordination for people who need extra support

At TwoHearts Recovery & Coordination, we provide specialised NDIS Support Coordination for participants with mental health and psychosocial disabilities.

Navigating the NDIS can feel overwhelming — especially when mental health challenges, crisis risk, or complex needs are involved. Our role is to bring clarity, structure, and steady guidance so participants can access the right supports and move toward greater stability.

Meet Your Support Coordinator – Chloe

Chloe is our dedicated Mental Health Support Coordinator, specialising in supporting children, young people, and families navigating complex behaviours and mental health challenges within the NDIS.

With extensive experience working with participants with psychosocial disabilities, Chloe brings clarity and confidence to what can otherwise feel like a confusing system. She has a particular passion for working alongside families supporting children with complex behavioural presentations, emotional regulation challenges, and mental health needs.

As a mother of three and currently studying her degree in Primary Education, Chloe understands firsthand the realities of parenting, advocacy, and supporting children through developmental and emotional challenges. This lived experience, combined with her professional knowledge, allows her to approach family-based support with empathy, structure, and practical understanding.

She works closely with:

  • Participants

  • Parents and carers

  • Schools and educational settings

  • Allied health professionals

  • Service providers

  • NDIS planners

Chloe is known for her steady communication, strong advocacy skills, and genuine care for the families she supports.

Her focus is not just on connecting services — but ensuring those services are working together in a way that feels manageable, consistent, and supportive for both the participant and their family.
